Photo ID, previous test results and discharge summaries, a list of the medication you take, and your policy if you are covered by insurance.

When to arrive

10 minutes before your appointment so we can set up your record. If you are running late, let us know and we will try to keep your slot.

Cancelling an appointment

Let us know at least 3 hours in advance by phone or on Viber — that way we can offer the slot to another patient.
